The Medical Home is a new model of care that has been sweeping across the U.S. in recent years, and it’s expected to be implemented in many other countries as well. The idea is that people with chronic illness will have their own primary care physician who helps them manage their health at home, and then the doctor sees them for checkups or emergencies when necessary. However, this model is not without its challenges. How can we make sure patients get the best possible care?
Advanced medical home Physicians are medical professionals who specialize in the care of patients with complex health needs. They work closely with other members of the healthcare team to provide comprehensive, coordinated care for these patients.

What is an Advanced Medical Home?
An Advanced medical home is a model of care that puts patients at the center of their care. Physician led teams coordinate all of the patientufffds health care needs, including:
-Preventive care
-Chronic disease management
–mental health and substance abuse services
-Rehabilitation services
-Pediatric and adult primary care
The Advanced Medical Home model of care is proven to improve quality and reduce costs. Advanced Medical Home Physicians (AMHP) is a national organization of physicians who are committed to providing high quality, coordinated, patient-centered care. AMHP works with Medicare and Medicaid to improve access to care for patients in underserved areas.
The Benefits of an Advanced Medical Home
The Advanced Medical Home (AMH) is a model of care that provides comprehensive, continuous and coordinated care throughout a personufffds lifespan. AMH physicians work together as a team to provide the best possible care for their patients.
AMH physicians coordinate all aspects of a patientufffds care, including preventive care, treatment of acute and chronic conditions and coordination of specialty care. AMH physicians also provide patients with education and support to help them make healthy lifestyle choices and better manage their own health.
The AMH model of care has been shown to improve the quality of care patients receive, while also reducing costs. In fact, Medicare beneficiaries who receive care from an AMH Physicians have been shown to have fewer hospitalizations and Emergency Department visits than those who do not receive care from an AMH physician.
If you are looking for a primary care physician in your area who participates in the Advanced Medical Home model of care, you can search the list of participating physicians on the Medicare website.
The Components of an Advanced Medical Home
The advanced medical home is a model of health care delivery that emphasizes care coordination and communication among a patientufffds health care team. The goal of the medical home is to provide high- quality, patient-centered health care.
The components of an advanced medical home include:
A team of health care professionals: Primary care physicians, specialists, nurses, and other health care providers work together to coordinate a patientufffds care.
A focus on quality: Advanced medical homes use quality measures to track and improve the care they provide.
Patient-centered care: Care is tailored to the individual needs and preferences of each patient. Patients are involved in their own care and are encouraged to take an active role in their health.
Care coordination: The medical home team coordinates a patientufffds care across all settings, including hospitals, clinics, and community resources.
Communication: The medical home team communicates regularly with patients, families, and caregivers to ensure that everyone is involved in the plan of care.
The patient-centered medical home Model
The Patient-Centered Medical Home Model of care is a team-based approach that puts patients at the center of their health care. Advanced Medical Home Physicians are proud to be leading the way in this new model of care.

The Team-Based Approach of an Advanced Medical Home
An Advanced Medical Home is a team-based approach to provide high quality primary care. The team includes the doctor, physician’s assistants, nurses, social workers, and other support staff. This team works together to coordinate your care and make sure you get the services you need when you need them. The Advanced Medical Home model has been shown to improve quality of care, lower costs, and improve patient satisfaction.
The Advanced Medical Home model is based on the “patient-centered medical home” model developed by the American Academy of Family Physicians. This model puts the patient at the center of their care and emphasizes prevention and early intervention to keep patients healthy. The Advanced Medical Home model builds on this by adding additional services such as behavioral health, pharmacy, home visits, and community outreach.
The Advanced Medical Home model is used by primary care practices across the country that are part of Medicare, Medicaid, and private insurance plans. In addition, many states and employers are now using the Advanced Medical Home model to improve the quality of care for their employees and residents.
The Use of Health Information Technology in an Advanced Medical Home
Health information technology (HIT) is defined as the application of information processing involving both computer hardware and software that deals with the storage, retrieval, sharing, and use of health care information, data, and knowledge for communication and decision making.1 The purpose of this article is to review the literature on the use of HIT in an advanced medical home (AMH).
AMHs are a model of primary care that emphasizes care coordination and communication among a patientufffds health care team, self-management support, comprehensive care, and population health management.2-5 AMHs have been shown to improve quality of care and decrease costs.6-9 Furthermore, AMHs have been found to be associated with better patient health outcomes10-12 and increased patient satisfaction.13-15 The use of HIT is integral to the success of AMHs.
The Coordination of Care in an Advanced Medical Home
The Coordination of Care in an Advanced Medical Home is a key factor in providing high quality medical care to patients. In an Advanced Medical Home, doctors and other health care professionals work together to provide comprehensive, coordinated care to patients. This coordination of care ensures that patients receive the best possible care, and that their health care needs are met in a timely and efficient manner.
The Advanced Medical Home model of care has been shown to improve the quality of care that patients receive, while also reducing the cost of health care services. In addition, the Advanced Medical Home model of care can help reduce the number of hospitalizations and emergency room visits, and can improve the overall health of patients.

The Payment Models for an Advanced Medical Home
The Payment Models for an Advanced Medical Home:
The Medicare Shared Savings Program (MSSP) is a physician-centered, quality-based care delivery and payment model created by the Affordable Care Act Under the MSSP, physicians can form Accountable Care Organizations (ACOs). ACOs are networks of doctors that coordinate and provide care to a defined group of Medicare patients. The goal of an ACO is to deliver high quality, coordinated care while reducing unnecessary costs.
There are two types of ACOs under the MSSP: Track 1 and Track 2. In both tracks, physicians receive a bonus for providing quality care (as measured by 32 quality metrics) while staying within a certain budget. In Track 1, physicians also assume downside risk; if their actual costs exceed their target costs, they must repay Medicare for a portion of the overage. In Track 2, there is no downside risk; however, physicians can earn a higher bonus if they meet or exceed both their quality and cost targets.
Advanced Medical Home (AMH) physicians participate in an ACO under either the MSSP or another CMS innovation model such as the CMS Pioneer Accountable Care Organization Model or the Medicaid Innovation Accelerator Program. To be eligible for participation in an ACO, AMHs must agree to use advanced health information technology (electronic health records), coordinate care among themselves and with other providers, and manage patientsufffd health risk factors. AMHs receive enhanced reimbursement for services rendered to Medicare and/or Medicaid beneficiaries who are attributed to their practice.
The Future of the Advanced Medical Home
The advanced medical home (AMH) is a model of primary care that puts patients at the center of their own care. This care is led by a doctor or team of physicians who work together with the patient to coordinate all aspects of their care. The goal of the AMH is to provide high-quality, coordinated, patient-centered care.
The AMH model has been shown to improve quality and reduce costs. In fact, a recent study found that Medicare patients who received care from an AMH physician had 10% fewer hospital admissions and 20% fewer emergency department visits than those who did not have an AMH doctor.
The future of the AMH is bright. As more and more physicians move into this model of care, it will become the standard of care for patients across the country.
Advanced Medical Home Physicians
Advanced Medical Home Physicians (AMHPs) are a quality improvement organization that works with doctors who care for Medicare patients in the home setting. AMHPs help these physicians improve the quality of care they provide and make sure that their patients receive the right care at the right time.
