What are the eligibility requirements for financial assistance?
To be eligible for financial assistance through Portneuf Medical Center, patients must:
-Be a resident of the United States
-Have a household income at or below 250% of the Federal Poverty Level
-Be able to provide documentation of income and expenses
-Not have insurance coverage that would cover the cost of their care
How can I apply for financial assistance?
Portneuf Medical Center offers financial assistance to patients who are unable to pay for their medical care. To apply for financial assistance, you will need to complete an application and submit it to the Financial Assistance office. You can get an application by calling the Financial Assistance office at (208) 239-1009.
What are the deadlines for applying for financial assistance?
The Portneuf Medical Center (PMC) financial assistance program is available to help patients with the costs of their care. To qualify, patients must first complete a financial assessment with a member of the PMC financial assistance team.
Patients who are found to be eligible for assistance will receive a discount on their bill based on their household income and assets. Discounts are available for both medical and hospital services, and can be applied to past due balances as well.

To qualify for financial assistance, you must:
-Be a resident of the United States or have status as a legal alien
-Demonstrate financial need by having an annual income at or below 200% of the federal poverty level
-Not have insurance coverage that would pay for all or part of the bill
If you meet these qualifications, you may be eligible for either complete or partial financial assistance. Complete financial assistance means that you will not be responsible for paying any part of your bill. Partial financial assistance means that you will be responsible for paying a portion of your bill, based on a sliding scale.
