-Thepatientufffds home must be determined to be an appropriate environment for receiving hospice services, taking into account factors such as the availability of 24-hour caregiver support and access to resources such as transportation and home health aides, if needed.
-The patientufffds caregiver(s) must be willing and able to provide the necessary support and assume responsibility for carrying out the plan of care under the supervision of our interdisciplinary team.
