Log into your account on the Anthem website
Go to the Anthem website and log in to your account. If you do not have an account, you will need to create one. Once you are logged in, click on the “My Profile” tab. From there, select “Cancel My Coverage.” You will be prompted to enter your reason for cancelling and to verify your identity by entering your date of birth and Social Security number.

1. Log in to your account at anthem.com.
2. Click on the “My Profile” tab.
3. Click on “Cancel Policy” under the “Policy Management” section.
