The Lipitz Center and its partner organizations will host a series of Webinars to provide detailed guidance for practices’ transformation into medical homes. The schedule will be available soon.
The Lipitz Center and its partner organizations will also host several two-day, in-person regional weekend Learning Collaboratives in 2010. The Collaboratives will provide in-person guidance on how to transform your practice into a medical home. Dates, locations, agenda, and cost for the Learning Collaboratives will be posted soon.
Learning Communities will provide practices with subsequent ongoing opportunities to network and learn from each other. More details to come!
