Practice Readiness Self-Assessment

TransforMED's "Medical Home IQ" (MHIQ) is a free, online tool that helps practices to self-assess their readiness to become qualified as Medicare medical homes. The MHIQ tool provides tips and resources for developing medical home processes and improving performance. Practices can also use this tool as a resource to educate staff members about the requirements and language associated with the Medical Home and use the tool to assemble documentation necessary for the CMS/NCQA medical home qualification process. The American College of Physicians' "Medical Home Builder" is another online practice support resource that provides an online evaluation of a practice's readiness to obtain NCQA qualification as a medical home ("Practice Biopsy") linked to resources selected based on the "biopsy" results.
