Guidance in Selecting Health Information Technology (HIT)

These selection tools assist practices in identifying health information technology that can support specific functions that medical homes provide.

  • The American College of Physicians’ (ACP) AmericanEHR Partners Program offers all health professionals guidance in the selection and implementation of practice-based EHR systems. Registration is free.
  • The American Academy of Family Physicians’ (AAFP) Center for Health Information Technology can help practices with the preparation, selection, implementation, and maintenance of HIT.
  • TransforMED’s Medical Home Marketplace makes it easy for practices to learn the benefits of products and to contact the appropriate vendors and/or account representatives.

Specific product information on HIT that has demonstrated the ability to generate comprehensive care plans from evidence-based guidelines will be available soon.
