About the Medicare Medical Home Demonstration (MMHD)

The Medicare Medical Home Demonstration (MMHD) will test a redesign of the primary health care delivery system to provide targeted, accessible, continuous and coordinated, patient-centered care to high-need populations.   

Section 204 of the Tax Relief & Health Care Act of 2006 mandates a demonstration in up to eight states to care for high-need Medicare beneficiaries. The three-year demonstration will provide reimbursement via a care management fee to physician practices for the unique services of a Medical Home.

The website provides a description of the Tier I and Tier II medical homes and provides details on patient eligibility, site selection, monthly medical home payments, and the payment process. Practices invited to apply for the demonstration can access application information, instructions, and the application form.
